Uniformed Services University Partners with NHTSA to Improve Post-Crash Survival on Highways

Prehospital Blood Transfusion to be the Main Focus

The Uniformed Services University (USU) in Bethesda, Maryland, through its National Institute for Defense Health Cooperation (NIDHC), is partnering with the Department of Transportation’s National Highway Traffic Safety Administration (NHTSA) to save lives on America’s roadways with prehospital blood transfusions.

USU has signed an Interagency Agreement with NHTSA aimed at supporting improvements in emergency medical services, 911, and post-crash care. This joint effort between USU’s NIDHC and NHTSA’s Office of Emergency Medical Services (OEMS) will improve motor vehicle post-crash survival rates while advancing our Nation’s medical readiness for other events in which trauma patients may require more robust prehospital care.

As NHTSA’s Chief Counsel, Peter Simshauser, indicated in his Road to Zero Annual Meeting remarks, “We know how critical post-crash care is to survival. Research shows that prehospital blood transfusion decreases mortality among trauma patients with severe bleeding by 37%. So, we’re partnering with the Department of War. This partnership will launch the largest federally-sponsored prehospital blood transfusion project ever undertaken, with the aim to create at least 25 new prehospital blood transfusion programs throughout the country in the next three years.”

Over the next several years, NIDHC and OEMS will facilitate the development of prehospital blood programs at sites across the U.S. Ultimately, this work will provide a clear path to greater and faster access to blood transfusion at the scene of a crash and to improved post-crash care on all of our Nation’s roadways.

The program will leverage NIDHC’s Joint Disaster Medicine and Public Health Ecosystem, which is composed of leading medical, academic, and research institutions from across the nation.

“This is not the first time, nor will it be the last, that the Departments of War and Transportation work together to translate lifesaving innovations from the military to the civilian medical community,” NIDHC Executive Director Dr. Jeff Freeman said. “NHTSA has a history of leaning forward in this way and countless lives have been saved because of it.”

OEMS 2025 Cost Collection Survey Notice

Ambulance Service Managers:

Minnesota Law 62J.49 requires the Office of Emergency Medical Services (OEMS) to maintain a financial data collection system for ground ambulance services licensed in Minnesota. We have made some adjustments to our data collection tool since the 2023 reporting cycle, but we have continued efforts to make the tool as easy to use as possible, while still collecting all the items required by law.

A memo with general guidance is below, the memo includes instructional documents and a link to the survey. All information must be submitted electronically via the online form.

All GROUND ambulance services are asked to complete the Cost Collection Survey by November 21, 2025.

NHTSA Launches New Resource Giving Emergency Responders Easy Access to Electric Vehicle Information

Comprehensive resource for ERGs and rescue sheets available

As part of the National Highway Traffic Safety Administration’s ongoing commitment to supporting first responders with timely and accurate information to enhance public safety, emergency responders have quick and easy access to electric vehicle (EV) Emergency Response Guides (ERGs) and rescue sheets through a new resource available on the NHTSA website.

Key features include:

Comprehensive Access: Emergency responders can quickly find and download ERGs and rescue sheets for EVs from all manufacturers.
Historical EV Files: Historical emergency response files for EVs have been transferred from the National Fire Protection Association (NFPA) website to NHTSA’s new platform.
Transition Away from NFPA Site: The NFPA will discontinue its ERG portal, and NHTSA’s new page will serve as the primary resource for these materials moving forward.

Standing Together in the Wake of Tragedy

To Minnesota Ambulance Association Members,

We are heartbroken by the devastating and senseless act of violence that took place last night, claiming the lives of Speaker Melissa Hortman and her husband, and injuring Senator John Hoffman and his wife, who remain in stable condition.

These elected leaders have been longtime advocates for stronger communities. We have had the honor of working alongside them in advancing public safety and supporting Minnesota’s EMS professionals. Their dedication to service was deeply felt — and will not be forgotten.

As members of the EMS community, we know all too well the pain and shock that violent incidents like this bring — not just to families and colleagues, but to entire communities. We stand in unwavering support of our fellow first responders and law enforcement officers as they investigate and respond to this tragedy.

We also ask our members — particularly those operating in the surrounding area — to remain alert, stay safe, and continue reporting any suspicious activity to authorities.

Please join us in keeping the families of Speaker Hortman and Senator Hoffman in your thoughts and prayers during this difficult time.

02 JUNE Legislative Update

MAA Members,

The legislature adjourned the regular session on May 19^th with kind of an uneventful close to the evening. They had only finished a few budget bills and 11 of the bigger ones including HHS Finance rolled over to what will be a “Special Session” that only the Governor can call. Over the last few weeks, we have been hanging around the Capitol providing technical support to legislators as they move to a position of leadership agreement. Just the other day the Health Finance bill which contain the EMS provisions released a signed and agreed to budget spread sheet. This spreadsheet only shows the funding and not the actual language. That language will be released closer to the Special Session. Below is what is in the Health Finance bill agreed to that we expect will be agreed to and passed during Special Session.

Here is quote Blois Olson a reporter had this morning: SESSION: “According to multiple sources the two biggest sticking points for calling a special session are the tax bill and a bonding bill. The DFL Senate caucus met for two hours Sunday which revealed a clear divide among members including leaders, and Chairs. The tax bill, which was agreed to on Friday lacks support from tax chairs and the bonding bill will require some serious negotiations from the Senate. Multiple legislators, including DFLers expressed a need for more engagement and weight of support from the Governor. Ultimately, it would seem compromise on bills that have bipartisan support could garner enough votes to pass, however legislators from the edges of the right and left remain barriers. The soonest a special session would be is likely Friday”.

Provisions in the Health Bill Spreadsheet:

They funded $16 million a year for EMS Rural Sustainability available until 2029.
Our bill to expand access of the EMR/EMT training to all ambulance services not just volunteer was included with an additional $400,000 for the next two years for a total of $800,000
The Ambulance Training Grants was funded at $2 million dollars and the money is available until 2029
The uncompensated care pool was funded for the next two years at $4,291,000 a year for two years for a total of $8,582,000
The rate increase was not funded. That was lost in the conversation around increasing the HMO surcharge and what’s happening at the Federal level.
Also, they did eliminate the broker prohibition and went to a statewide broker for Non- Emergency Medical Transportation. This is an area we will need to monitor closely

So, overall, with the legislative session ending with no budget deals and the pending Special Session the MAA agenda around workforce and funding continues to be supported at the legislature. My rough math shows that we secured close to $27,500,000 of funding the next two years of the budget. That’s a total of $62 million into EMS the last 3 years.

I want to extend a special thanks to the MAA Board, Legslative Committee and most importantly Samantha and Mark who turn our updates and documents around quickly so the work can be done at the Capitol. Also, Erik Simonson and the Coalition of Greater MN Cities has been an amazing partner in this work, and I look forward to continuing that relationship.

We will have a more detailed update once the Special Session takes place and we see the final language.

We can prevent Minnesota ambulance services from flatlining

In the final days of the legislative session, lawmakers must act to shore up EMS services — especially in rural areas.

When you call 911, you expect help to arrive — fast. You expect trained professionals to show up with the equipment and expertise to save lives. But that expectation is at risk across Minnesota, especially in rural communities, because our emergency medical services (EMS) system is being pushed to the brink.

Between the two of us — one leading the Minnesota Ambulance Association and the other directing Perham Area EMS — we’ve both seen up close just how strained ambulance services are across the state. The time for action is now. As lawmakers finalize the state budget in the final days of the 2025 legislative session, they must prioritize sustainable EMS funding. Without it, lifesaving care will be at risk in more communities across Minnesota.

FY25 Safe Streets and Roads for All (SS4A) NOFO Now Open

The Department of Transportation (USDOT) has released the FY25 Notice of Funding Opportunity (NOFO) for the Safe Streets and Roads for All (SS4A) grants. The estimated $900 million in SS4A funding will provide resources for regional, local and Tribal initiatives focused on preventing roadway deaths and serious injuries. Applications for Planning and Demonstration Grants and Implementation Grants are due Thursday, June 26, 2025, by 5:00 PM ET.

The National Highway Traffic Safety Administration (NHTSA) Office of EMS encourages EMS agencies and 911 centers to apply for funding independently or collaborate with local, regional, health, planning or transportation partners.

Paramedic Safety, Resilience, and Resources in Minnesota: Honoring and Protecting Those Who Serve

Emergency Medical Services (EMS) professionals in Minnesota face unique challenges in their daily work, responding to crises that demand skill, compassion, and resilience. The safety and well-being of paramedics and EMTs are paramount. However, the profession continues to experience high rates of injury, burnout, and mental health struggles. As we work to support our frontline responders, we must address both immediate safety concerns and long-term resilience strategies while also honoring those who have made the ultimate sacrifice.

Safety Challenges in EMS

Minnesota’s EMS workforce operates in environments where physical and psychological hazards are prevalent. According to the National Institute for Occupational Safety and Health (NIOSH), EMS workers experience an injury rate of 34.6 per 100 full-time workers, nearly three times the national average for all occupations (NIOSH, 2021). These injuries result from patient handling, vehicle accidents, and assaults, underscoring the need for ongoing safety measures, proper equipment, and workforce training.

The Minnesota Department of Health (MDH) has emphasized the need for expanded access to personal protective equipment (PPE), enhanced de-escalation training, and improved vehicle safety standards to protect paramedics on the job (MDH, 2023). Furthermore, legislation such as HF 1492, introduced in Minnesota, seeks to improve protections and resources for first responders, including PTSD support and critical incident debriefing programs (Minnesota Legislature, 2024).

Building Resilience in EMS Professionals

Building resilience in EMS requires both individual and organizational support. Recent studies published in the Journal of Emergency Medical Services (JEMS) suggest that structured peer support, wellness programs, and mental health resources significantly reduce burnout rates among paramedics (JEMS, 2022).

Additionally, organizations like the Minnesota Ambulance Association (MAA) and the National Association of Emergency Medical Technicians (NAEMT) advocate for initiatives such as mandatory rest periods, comprehensive mental health services, and financial support for EMS personnel facing hardship (NAEMT, 2023).

Honoring Our Fallen: The MN State EMS LoDD Memorial Foundation

While we strive to improve safety and resilience, we must also recognize those who have given their lives in the line of duty. The Minnesota State EMS Line of Duty Death (LoDD) Memorial Foundation was established to ensure that the sacrifices of EMS professionals are never forgotten. Founded by Adam Harvey, a 20-year EMS veteran, the foundation is working to build a permanent memorial at the Minnesota State Capitol to honor fallen EMS personnel.

To date, the foundation has garnered support from major organizations, including Allina Health, the Minnesota Ambulance Association, the IAFF, and the Minnesota Nursing Association. The project, which has already raised over $10,000, continues to seek additional funding through grants and corporate sponsorships. Honoring our fallen not only pays tribute to their service but also reinforces the urgency of protecting current EMS professionals from preventable harm.

A Call to Action

Ensuring the safety, resilience, and well-being of Minnesota’s EMS professionals requires a collaborative effort from government agencies, healthcare organizations, and the communities they serve. We must continue advocating for legislation, funding, and policies that protect those who dedicate their lives to saving others. Supporting initiatives like the MN State EMS LoDD Memorial Foundation ensures that their sacrifices are never forgotten.

By investing in safety measures, mental health resources, and memorial efforts, we honor the past while strengthening the future of Minnesota’s EMS workforce. Together, we can ensure that our paramedics and EMTs receive the recognition, support, and protection they deserve.

Special Elections & other Legislative Updates from MAA

As Minnesota prepares for a dynamic 2025 legislative session, several special elections and legal challenges are shaping the political landscape. With key races in Districts 40B and 60, along with potential contests in 54A and 47, the outcomes could significantly influence state leadership. The Minnesota Ambulance Association (MAA) is advancing a robust legislative agenda to strengthen EMS funding, training, and sustainability—be sure to continue reading for additional details.

Special Elections 2025:

As we all watched for legal challenges and chaos in the Federal election we seemed to have our own issues in MN this past November.

In District 40B, the Shoreview, North Oaks, Arden Hills the election was called due to the elected candidate being found to have not resided in the district and had rented an apartment he never stayed in. That candidate resigned and the seat opened. Five Democrats and one Republican have filed for the seat, that triggered a primary election to be held on January 14^th ahead of the general election on January 28^th. That district is expected to stay DFL.

In Senate District 60, the special election was called due to the death of Senator Kari Dziedzic, the former majority leader of the MN Senate. 10 Democrats and three Republicans filed for that seat. Neither of the current House members filed for the seat. This special election will also have a primary on January 14^th with a Special election on January 28^th.

And keep in mind with those two special elections, there remain two more potential special elections. In House District 54A, the Scott County race where 20 ballots were lost and the winning margin was, but 14 votes was challenged in court by the House Republicans. The current winning candidate Brad Tabke wants the election certified so he can be allowed on the House floor when the session starts. This case is now in the hands of the judge who is expected to rule soon. The other potential special election is in Senate District 47 (Woodbury) where Senator Nicole Mitchell appeared in court recently as a settlement conference began related to her burglary charge. From these legal negotiations numerous things could happen in this one including Mitchell continuing her term, a voluntary resignation as part of a settlement or removal from office if she is convicted of a felony.

With all this in mind as I write this pre-session update the legislature will start on January 14^th with a 33-33 tie in the Senate, and a 67-66 Republican majority in the House. Either way the legislature will begin on January 14^th as we await special elections and court rulings.

The MAA 2025 Legislative Agenda:

At the direction of the MAA Board and Legislative Committee we have drafted up a few pieces of legislation to start the 2025 legislative session.

1.        We drafted up a bill to increase Medicaid rates for all ambulance services. In talking with DHS and our bill author we have not decided on a percentage yet. That number will be considered as budget negotiations at the legislature progress this session.
2.        We drafted up a bill to expand access to the EMT training reimbursement program managed by the Office of EMS, which continues to have a balance every two years of non-encumbered funding. This bill would increase reimbursement for EMT initials and refresher courses, remove the word “volunteer” to open the pool to more ambulance services in rural MN. And, most importantly it adds our important First Responders who work on ambulance services to the reimbursement formula.
3.        Mark and I have also been working with the revisors office to draft up the continuation of the very successful Paramedic Scholarship program which is managed by the MN Office of Higher Education. That office has collaborated and worked with the MAA and the Office of EMS the last two years to market and get new paramedics in your rigs. We look forward to the opportunity to work on extending and expanding access to this for the members.
4.        Also, the MAA Legislative Committee has been working on a draft bill regarding a EMT apprenticeship program. We are awaiting some more technical feedback and should have that bill ready to be drafted this next week. Thanks to Mark Jones, Dylan Ferguson and Tom Fennel who have been working hard on this important workforce legislation.
5.        We also continue to work with the Greater Cities Coalition on some opportunities to secure sustainability funding for services with lower EMS volume and continued financial viability issues. Although $24 million was secured last session additional funding and a more uniformed all-inclusive program needs to be established regardless of your legal status as a private, non-profit, hospital based, Governmental or Tribal entity.

The New Office of EMS:

The new Office of EMS began on January 1, 2025, and the transition seems to have been Seamless as it continues to be led by Dylan Ferguson. I would like to extend a sincere thank you to Director Ferguson for attending numerous meetings around the State and MAA Board meeting to provide Transparency on the entire process. Marburg Virus Disease: New EMS Guidelines

Viral Hemorrhagic Fevers (VHFs) are severe illnesses caused by a range of viruses, some of which have high fatality rates. Although cases are rare in the United States, these viruses are becoming more prevalent internationally. Examples of VHFs include Marburg virus disease (MVD), with a fatality rate averaging around 50% (ranging from 24% to 88%), Lassa fever, with a hospital fatality rate of 15% to 20% (and as high as 50% in pandemic settings), Ebola virus disease (EVD), and Crimean-Congo hemorrhagic fever (CCHF). The recent national attention surrounding a fatal Lassa fever case in Iowa underscores the importance for EMS clinicians to stay informed about VHFs.

The National Highway Traffic Safety Administration (NHTSA) Office of EMS strongly encourages EMS clinicians to familiarize themselves with VHFs, including where they occur, how they spread, what questions to ask patients, and how to protect themselves. The National Emerging Special Pathogens Training & Education Center (NETEC) provides guidance for the management of VHFs and has recently updated the MVD NETEC guidelines for EMS clinicians, in October 2024 to align with current CDC recommendations. These guidelines for EMS clinicians emphasize the "Identify, Isolate, and Inform" paradigm, previously used for diseases like Ebola and mpox.

Emergency responders are encouraged to learn the signs and symptoms of VHFs and to assess patients with travel-related questions when appropriate. While most EMS clinicians in the United States are unlikely to encounter a VHF case during routine operations, international outbreaks heighten the risk of encountering an infected patient who may have traveled from an affected region. EMS agencies serving areas with international ports of entry should be especially vigilant.

In one year, the program provided over 1,700 primary care visits for 825 patients at 17 different sites. CY 2025 Physician Fee Schedule Final Rule

CMS issued the CY 2025 Physician Fee Schedule (PFS) final rule that announces finalized policy changes for Medicare payments under the PFS and other Medicare Part B payment policy issues, including ambulance services. For CY 2025, we proposed to modify the definition of advanced life support level two (ALS2) at § 414.605 by adding the administration of low titer O+ whole blood transfusion (WBT) to the list of ALS2 procedures. We also solicited comments on whether we should add alternative blood product treatments, such as the administration of packed red blood cells (PRBCs) and plasma, to the list of ALS2 procedures.

After consideration of public comments, and considering further that the indications for use, storage requirements, administration, monitoring, and training for the use of PRBCs and plasma are similar to those previously described for WBT, we are finalizing our proposal to modify the definition of ALS2 at §414.605 by adding the administration of prehospital blood transfusion (PHBT) which includes low titer O+ and O- whole blood transfusion therapy, PRBCs, plasma, or a combination of PRBCs and plasma. A ground ambulance transport that provides one of these PHBTs would itself constitute an ALS2 level transport.

MAA Hosts Roundtable in Duluth On Challenges Facing EMS in Minnesota

The latest in a series of MAA roundtable events was held on Friday, Oct. 25 at Mayo Clinic Ambulance in Duluth.

A great crowd of local residents, EMTs, ambulance service providers, elected officials and staff were in attendance for the conversation. Panelists were MAA President Michael Juntunen, Cloquet Area Fire Chief Jesse Buhs, and Minnesota Rural Health Association Executive Director Mark Jones, with the discussion moderated by Kate Raddatz of McClung PR.

Topics included issues facing rural EMS, the role ground ambulance services play in the healthcare system, the impact recent state legislation has had, and recommendations for a sustainable future and continued, equitable access to care for all Minnesotans. Elected officials in attendance included State Rep. Natalie Zeleznikar (HD 3B, Hermantown) and State Rep. Roger Skraba (HD 3A, Ely), as well as Jacob Warkentin from the office of Congressman Pete Stauber (MN-8). Media in attendance included WDIO-TV, Fox 21, and the Star Tribune Duluth bureau. Newly appointed director of the Office of Emergency Medical Services Dylan Ferguson was also there for the conversation.

The next MAA roundtable will be held in Dodge Center on Wednesday, November 20 at 1:00 pm.

CMS is proposing to modify the definition of advanced life support level two (ALS2) at 42 CFR §414.605 by adding the administration of low titer O+ whole blood transfusion (WBT) to the list of ALS2 procedures. For patients in hemorrhagic shock, the use of WBT has demonstrated a survival benefit when compared to traditional hemorrhagic resuscitative therapy. Under this proposal, a ground ambulance transport that provides WBT would itself constitute an ALS2-level transport.

We are also seeking comments on whether we should add alternative blood product treatments such as the administration of packed red blood cells and plasma to the list of ALS2 procedures. We are aware that some established Emergency Medical Services (EMS) systems may already provide WBT to treat patients in hemorrhagic shock, while other jurisdictions, including those in rural areas, may rely on alternative blood product treatments.

We encourage you to submit comments on this proposal by September 9, 2024, to the website provided in the CY 2025 PFS proposed rule.

NHTSA’s Office of EMS will host a 4-hour workshop to discuss and prioritize the responses to the EMS Education Agenda 2050 (EEA2050) Request For Information (RFI). Published in the Federal Register in October of 2023, the RFI remained open until the end of March 2024. It requested responses to twenty-two questions from all sources about the vision of EMS Education in the United States by the year 2050.

The EEA2050 workshop will take place as a pre-National Association of EMS Educators (NAEMSE) Symposium workshop.

As we wrap up the legislative session, we want to keep you informed about the recent changes affecting emergency medical services in Minnesota. Below, you'll find a summary of the bills that were passed and those that did not make it through.

We will share additional information about implementations and impact in the coming weeks and months for those initiatives that continue to take shape.

Bills Passed by Legislature

Office of EMS, alternative response model, staffing, emergency ambulance aid

1 May - Legislative Update

MAA Membership:
The Minnesota Legislature is three weeks away from its mandatory adjournment date of May 20, 2024. To add intrigue to the session, yesterday’s floor sessions (April 30) marked the 108^th legislative day of the biennium. By rule the legislature can only meet in regular session (floor sessions) on no more than 120 days. Between now and May 20^th, the legislature only has 12 days left to hold floor sessions; this leaves quite a bit of work to get done with limited time to do so!

Recent developments regarding Senator Nicole Mitchell (DFL – Woodbury), who was arrested and charged with first-degree burglary, have raised questions about how the session will proceed and ultimately end. The Senate has spent a good amount of time during floor sessions over the past week debating how to best handle Senator Mitchell’s involvement with voting during the remainder of the session. The DFL party has a one-vote majority. If Senator Mitchell does not cast a vote, it is possible that remaining bills will fail due to a 33-33 tie vote. We are hopeful that EMS funding is a priority to all legislators and will receive bi-partisan support if brought to the floor.

The House of Representatives has been very active in moving bills out of committee and off the floor. We assume they will continue to be active in moving bills and await action from the Senate. Unlike the number of days remaining to complete the work of the 93^rd Legislature, the amount of activity will not be limited.

Priority Summary

Emergency Ambulance Service Aid

HF3992 (Lislegard) has been the primary ambulance aid bill since it’s introduction on February 19^th. The bill has made all deadlines and continues to make its way through the process. It currently sits in the Ways and Means Committee awaiting a hearing to set the funding level. The Ways and Means Committee serves as the committee which controls the money. As you may recall, the Governor proposed $10 million in funding for ambulance services in the northeast and central regions.

SF3886 (Hauschild) is the companion to Rep. Lislegard’s bill. The bill was heard in Taxes back on March 6^th. The committee had a good conversation around the need for the funding. The bill was laid on the table for further discussion, which is where it currently sits.

Representative Huot introduced the governor’s proposal in HF5275, which was never heard. HF5399 (Backer) was introduced as a companion to SF5433 (Rasmusson) to provide aid to ambulance services based on a formula that takes into account geographic area served and run volume. The MAA and Coalition of Greater Minnesota Cities (CGMC) have met with Rep. Backer and Senator Rasmusson several times over the past few weeks. Our efforts have been focused on finding common ground between all the bills, leadership’s budget targets, and ultimately the needs of ambulance services.

Please contact your legislators to request that they support this one-time aid for ambulance services. There is quite a difference between the $10 million recommended by the Governor and the $120 million originally requested. In your conversations with legislators, please encourage them to support funding at the $120 million level. Receiving the full funding amount is unlikely but that does not mean we shouldn’t keep our message focused on the full need of the EMS system in Minnesota.

Establishment of the Office of EMS

The establishment of the Office of EMS language is in the Omnibus Health Finance bills of both bodies. HF4571 (Liebling) and SF4699 (Wiklund) contain identical language including an amendment that the MAA brought forward to ensure that the Deputy Director of Medical Services is a physician.

The original bills, HF4738 (Huot) | SF4835 (Seeberger), are moving along separate from the Omnibus Health Finance bills. The Huot bill is awaiting a hearing in the Ways and Means committee. It is very likely that this and Rep. Lislegard’s emergency aid bill, HF3992, will be taken up by the committee together to establish funding for both.
- HF4738 (Huot) | SF4835 (Seeberger) Establishment of the Office of EMS – Both bills have been heard in the Health committee of their body. HF4738 was heard in State and Local Government Finance and Policy and rereferred back to the Health Finance and Policy Committee. SF4838 was heard in State and Local Government and Veterans Committee and laid over for possible inclusion in an omnibus bill.
- Both bills have also had the Alternative EMS Response Model Pilot Program amended to the bill which would establish a “sprint medic model” to be tested in selected communities.
- $6 million is budgeted for the transition from the EMSRB to the Office of EMS and for the sprint medic model.
- The MAA had initially opposed the bills as physician involvement was minimal. After the authors amended their bills to include a physician in a director role, the Association moved their position to neutral.

Please reach out to your legislators to share the importance of keeping EMS a physician-led industry in Minnesota.

Licensing and Staffing Bill

The licensing and staffing language is included in the Omnibus Health Finance bills of both bodies. HF4571 (Liebling) and SF4699 (Wiklund) both include the CFRN and CEN language along with alternative staffing and the removal of the restriction on the large cities regarding staffing variances. The bills update educational requirements and extend the period for which a person with a lapsed license can apply for reinstatement.
- HF4600 (Huot) | SF4697 (Seeberger) Licensing and Staffing Bill – HF4600 was heard in the House Health Finance and Policy Committee on March 21st. The bill was amended to include certified flight registered nurses (CFRN) and certified emergency nurses (CEN) as ambulance service personnel. The amendment also removes the restriction on the metro counties, and the cities of Duluth, Mankato, Moorhead, Rochester, St. Cloud with regards to the staffing variances. The intent of the amendment is to allow all ambulance services to apply for a variance to use alternative staffing. The bill, as amended, was laid over for possible inclusion in an omnibus health bill.
- SF4697 was heard in the Senate Health and Human Services Committee on March 15^th. The bill does not include the same language regarding certified flight nurses, certified emergency nurses, or the removal of the metro and urban cities restriction.

According to the National Harm Reduction Coalition, “harm reduction is a set of practical strategies and ideas aimed at reducing negative consequences associated with drug use.” This can include improving access to naloxone (Narcan) to prevent death from opioid overdose, providing sterile syringes to reduce the spread of HIV and hepatitis C, and fentanyl testing programs, among other types of help.

EMS clinicians are often on the frontline of addiction and the unintended consequences of both legal and illicit drug use and substance use disorder (SUD). In addition, drug use greatly impacts the larger context of our nation’s roads and highways through impaired driving and the resulting risks to the impaired driver, other drivers, passengers, bicyclists, and pedestrians. A 2022 NHTSA study of seven trauma centers around the U.S. found that nearly 56% of people injured or killed on roadways tested positive for one or more drugs, including alcohol. The most prevalent drug category was cannabis, and opioids made up 8.5% of cases at the trauma centers.

Susan Long, the President of EMS for Allina Health EMS, brings to the position an impressive 43 years of dedication and experience in EMS. NIOSH Advisory on PPE Recommendations for Responding to Chemical Suicide Incidents

The National Institute for Occupational Safety and Health (NIOSH) has partnered with the leading experts from the Pennsylvania Association of Hazardous Materials Technicians to release a new Safety and Health Advisory. The Personal Protective Equipment Recommendations for Response to Chemical Suicide Incidents advisory aims to provide responders with critical guidelines on the recommended personal protective equipment (PPE) when attending to chemical suicide scenarios. This advisory can be used as a guideline to provide personal protective equipment recommendations for first responders to minimize the likelihood of exposures, injuries and fatalities during chemical suicide incidents. Minnesota Paramedic Scholarships

The Minnesota Office of Higher Ed has published a website outlining preliminary details of the MN Paramedic Scholarship Program.

The Minnesota Paramedic Scholarship will begin in Fall 2024

The Minnesota Paramedic Scholarship is a temporary scholarship program for Minnesota residents completing a paramedic diploma or degree. It was passed during the 2023 Minnesota Legislative session to increase the number of licensed paramedics in Minnesota. A one-time appropriation for $3 million will provide up to 600 scholarships of $5000 each starting Fall 2024 to be awarded by June 30, 2026.

Medicare Ground Ambulance Data Collection System: Office Hours Session — April 27

Dear Ground Ambulance Providers and Suppliers,

Starting January 1, 2023, ground ambulance organizations selected in Year 1 and Year 2 are required to report cost, utilization, revenue, and other information to CMS within 5 months after your data collection period ends. Organizations that fail to report may be subject to a 10% payment reduction.

Ground ambulance organizations selected in Year 3 and Year 4 will begin collecting data in 2023 and reporting data beginning in 2024.

Medicare Ground Ambulance Data Collection System: Office Hours Session — April 27

Thursday, April 27 from 2–3 pm ET

We divided the office hours session into 2 topics:

2–2:30 pm: CMS will answer live questions on the data collection requirements (such as what am I required to collect and when do I report?). This part of the session is for all selected organizations in Years 1–4.
2:30–3 pm: We’ll answer live questions about registering and requesting access to the Medicare Ground Ambulance Data Collection System portal. This part is for selected organizations in Years 1–2 who are required to report beginning in 2023.

EMS Medical Directors & ambulance service leaders urge lawmakers to take action and bolster critical ambulance services

St. Paul — EMS providers across Minnesota are confronting serious challenges as they face staff and volunteer shortages and increased demand for emergency medical services, EMS leaders said at a State Capitol press conference today.

Advocates gathered in St. Paul to meet with lawmakers and encourage action this year to bolster ambulance services.

In the 1980s, the federal government began decreasing financial support for ambulance services, leaving local services responsible for funding. Today that contributes to financial pressures – the price tag to operate one ambulance with 24/7, 365 on-call access is close to $1 million annually. And that’s with many rural EMS programs dependent on “volunteers,” who are often modestly compensated but not at levels truly commensurate with the time they give and the services they provide.

Each year ambulance service providers throughout the state respond to nearly 715,000 calls for service annually using 804 individual ambulances. All Minnesotans count on EMTs and Paramedics throughout our state to provide not only transportation, but high-quality out-of-hospital care to keep Minnesotans safe and healthy.

Dr. Aaron Burnett practices Emergency Medicine at Regions Hospital in St. Paul and Lakeview Hospital in Stillwater. He serves as an EMS Medical Director for several agencies in the Twin Cities East Metro. He believes in the importance of strong pre-hospital patient care and has witnessed the building pressure on EMS and ambulance services.

“This is probably one of the most important things coming up in the next 10 to 20 years as our population gets older and older,” said Dr. Burnett. “As a state we shouldn’t wait until there’s a public outcry because people can’t get an ambulance in a timely manner. Every day citizens in Minnesota rely on EMS services for emergency care. We owe it to those patients to ensure our EMS system is appropriately supported and funded at the state level.”

Dylan Ferguson became executive director of Minnesota’s Emergency Medical Services Regulatory Board in February 2022. His organization has compiled data showing the workforce shortage in EMS, as workers leave the field in pursuit of better pay and working conditions.

Of EMTs not renewing their license and leaving the profession, nearly 65% are under the age of 40
In 2021, 4,474 certifications expired while 1,558 certifications were issued – a deficit of 2,916 EMS providers
39% of EMS providers leaving the profession said their decision was influenced by low pay

“Minnesotans depend on emergency medical services every day,” said EMSRB executive director Dylan Ferguson. “As the state's lead agency for emergency medical services we are working together with our fellow government agencies, officials, and health care and community leaders to reverse these concerning trends and ensure a sustainable system.”

EMS leaders shared solutions to the workforce and funding challenges and encouraged lawmakers to talk with providers in their areas to learn more about these issues:

Increased reimbursement levels from the Federal Government and State Government by realizing opportunities to leverage more Federal dollars for Governmental payers. Legislation will be introduced soon to address opportunities.
A focus on improving retention of the current EMS workforce, such as the income tax subtractions for Volunteer Fire & Rescue Workers in HF 98
Development and support for future EMS workforce, including Establishment of a Paramedic Scholarship Program, HF 165, and funding for current and new EMTs under HF 141

Stevens County EMS Director Josh Fischer noted that navigating increased gas costs has been particularly difficult at the same time as a worker shortage.

“We still have to respond, we still have patients that need to be cared for and transported and we can’t simply increase what we bill because what we bill doesn’t correspond with what we actually get paid when we’re talking about Medicare and Medicaid reimbursement," said Fischer. “We need our elected officials at all levels to help us pursue sustainable solutions to these challenges.”

Medicare Add-Ons Extended to 2024

Congress has extended the Medicare add-ons for ground ambulance services by two years, until 2024 (Section 4103) as part of the Omnibus package.

It takes a village and we want to thank everyone who took part in the letter writing campaign, attended hill visits, or reached out to your elected officials and encouraged them to support ground ambulance services.

NOTE: The Omnibus bill is more than 4,000 pages long. The American Ambulance Association advocacy team is still in the process of reviewing the full text and will provide a full update in a webinar in early 2023. We will share a link to that webinar when it is available.

MAA's Rural Resource Center Releases 2022 Gaps Analysis
